How to Apply
The Financial Assistance Application is available for download here. If you prefer, you can pick up a copy from our Member Service Desk.
To submit your application:
- Complete the application thoroughly and accurately
- Attach the following required documents
- Your most recent 1040 tax form
- Or your two most recent pay check stub
- Any other information which demonstrates your need for special consideration
- Return your fully-completed application to our Member Service Desk.
Please submit your application at least two weeks before you plan to begin any program or receive membership.
